It’s normal for a provider’s office to verify your plan benefits. Here are answers to commonly asked questions:

What's the name of your insurance?

Your Answer: I have health benefits through my employer’s self-funded insurance plan. That means my employer pays provider claims directly. Imagine360, a third-party administrator, processes the claims. The plan allows me to see my provider of choice.

We don’t recognize your insurance, so how can we confirm your benefits and eligibility?

Your Answer: The plan allows me to see my provider of choice. Rates are typically in line with reimbursements you’d get from other networks. Please take a minute to call the provider number on my Benefits ID card. An Imagine360 representative will quickly explain how easily my benefits plan works.

What network does your plan use?

Your Answer: The plan allows me to see my provider of choice. All your office has to do is bill the plan. Rates are typically in line with reimbursements you’d get from other networks.

Where do we submit your claims?

Your Answer: The electronic Payor ID is 48143. Or mail them to: Imagine360, PO Box 749075, Dallas, TX 75374. This information is also on my Benefits ID card.

If a provider still wants more information or asks you to pay more than your applicable copay or deductible at the time of care, call the member number on your Benefits ID card. We can talk with them for you.
